Please answer a few questions, so I can arrange a time for your class. Skip any questions that don't apply to you.
50-minute classes meeting three times a week 75-minute classes meeting twice a week no preference

Yes. I consider this to be more important than avoiding student conflicts. Yes, but I might reconsider as a last resort to avoid conflicts for the students. Either way is fine with me. No, I would rather teach the two classes on different days.
back-to-back classes one free period between classes as much time as possible between classes
Please tell me about any demands on your time that will affect your teaching schedule. In particular, list any classes or seminars (between 8 a.m. and 4:30 p.m. weekdays) that you want me to schedule your class around. If you can think of any courses in other departments that should not conflict with yours, that would also be good to know.
If you need a classroom with more than 20 seats or audio/visual equipment beyond an overhead projector, please explain here.
If you find this form inconvenient or cumbersome to use, please tell me why. I will try to make improvements for future semesters.
