Faculty Conflict of Interest/Commitment Reporting

The university conducts the annual faculty conflict of interest/commitment reporting every spring. Collection generally runs from mid-April to early May. It is coordinated in the Mathematics Department by Cathy Stevens. Important: The Office of Sponsored Programs will NOT accept proposals for sponsored research from faculty who have not completed the disclosure form(s)!

Paper Submission

Submit Parts I and II to Cathy Stevens. The forms can be obtained from Cathy, or you can download a PDF version from the University Faculty website.
